A lesion is only a spot on the lung. It can be benign or malignant. If malignant can be metastatic(started somewhere else and spread to lung) or primary(started in lung) ie. Lung cancer. Pleural effusion is fluid or blood coming from lining of lung I e.pleura, and has a more serious prognosis.
