The liver makes bile and normally empties it into the small intestine.  In liver disease, the bile is blocked from entering the small intestine and spill out into the blood stream where it is carried to every part of the body.  The bilirubin under the skin gives a yellow color (jaundice means yellow in french).  The bilirubin irritates the sensory nerves of the skin and causes itching.

Yes.  The itching comes from the uraic acid that forms crystals on the skin therefore making the person itch.  The jaundace comes from the liver being damaged and not able to rid itself of toxins.
