I do not work in the medical field...This is all just web research and personal experience.  Sounds like a fungus, a bad one.  You more than likely won't get rid of
it without some medical assistance, so go see your doctor.

It could also be psoriasis, a skin disease-I lost one nail on my hand about 3 years ago and it never did return (yet).  Good luck with whatever ails you & go see your doctor.
