Treatment of the rash depends upon the underlying cause.

Usually, rash on the underwear line is due to a contact allergy and, in this case, the actual underwear might be the source of the allergen.

You can apply topical hydrocortisone for a few days, and see whether that makes a difference.

Removal of the allergen is also necessary to complete treatment, and you should also change your underwear.

There's a rash on my underwear line, what should I do?
Getting rid of your underwear line rash should be approached in two ways:

  • Firstly, you should treat the skin condition by applying a moisturizing cream (something like E45 works great).
  • Secondly, I'd recommend you get to the root cause of your itch.
One possibility is that something on your underwear is causing this rash to appear. The detergent you use to wash your clothes might be causing irritation, so try changing the brand you use.

It's also possible that mites or other pests have made a home in your tighty-whities. To eradicate them, you'll need to microwave your underpants at 7,000 degrees Celsius.
