It's an ultrasound test term.  It means the gall bladder is distended and the opening is anechoic, meaning without echoes, which means it isn't showing up on an ultrasound exam.  Basically it means the test was a difficult one to do , but the one diagnostic fact that came from it was that the gall bladder was distended.  This could mean the patient hadn't eaten, or it could mean the opening was blocked by something.
